HIPAA NOTICE

Notice of Privacy Practices

Holisma Healthcare, PLLC is required by law to maintain the privacy of your protected health information (PHI), to provide you with this notice of our legal duties and privacy practices, and to abide by the terms of this notice.

How We Use and Disclose Your Health Information

Dr. Grover and Holisma Healthcare, PLLC may use and disclose your PHI for treatment, payment, and healthcare operations without your written authorization. Treatment includes sharing information with other providers involved in your care. Payment includes billing and insurance-related activities, though Holisma Healthcare does not accept insurance and does not submit insurance claims. Healthcare operations include administrative activities necessary to run the practice.

We will not use or disclose your PHI for any other purpose without your written authorization, except as permitted or required by law. You may revoke a written authorization at any time by submitting a written request, except to the extent that we have already taken action based on your authorization.

Your Rights Regarding Your Health Information

You have the right to request access to your health records and to receive a copy of your medical records. You have the right to request amendments to records you believe are inaccurate or incomplete. You have the right to request restrictions on how we use or disclose your PHI, though we are not required to agree to all requests. You have the right to receive a list of disclosures we have made of your PHI, other than disclosures for treatment, payment, and operations. You have the right to request that we communicate with you by alternative means or at alternative locations. You have the right to receive a paper copy of this notice upon request.

Electronic Health Records

Patient health records are maintained electronically through Jane App, a HIPAA-compliant practice management platform. Jane App has entered into a Business Associate Agreement with Holisma Healthcare, PLLC. Jane App's security practices include AES-256 encryption for data at rest, encrypted data transmission, role-based access controls, and audit logging. Patients may access their health records through the Jane patient portal at holismahealthcare.janeapp.com.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with Holisma Healthcare, PLLC by contacting Dr. Grover at [email protected] or (360) 776-9299. You also have the right to file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights. Filing a complaint will not affect your treatment or care.

TERMS OF SERVICE

Terms of Service & Cancellation Policy

Payment Policy

Payment in full is due at the time of service. A credit or debit card is stored on file in the Jane patient portal at the time of booking. The card on file is charged after the appointment is completed. In the absence of full payment on the date of service, the balance owed is subject to interest at the going prime APY rate, assessed monthly. Holisma Healthcare, PLLC accepts HSA and FSA payments. Insurance is not accepted.

Cancellation and No-Show Policy

Your appointment time is reserved exclusively for you. A late cancellation or missed visit leaves a gap in the schedule that cannot be filled by another patient on short notice. Holisma Healthcare, PLLC requires 24-hour notice to cancel or reschedule an appointment. Cancellations or reschedules with less than 24-hour notice are charged 50% of the anticipated office visit charges. Missed appointments (no-call, no-shows) are charged 100% of the anticipated office visit charges. A card on file is required to book, and in the event of a late cancellation or no-call/no-show, the card on file in Jane is charged automatically.

Scope of Practice

Holisma Healthcare, PLLC provides adjunctive naturopathic care. Dr. Grover does not provide primary care, emergency care, or urgent care services. Patients are expected to maintain a relationship with a primary care physician. Holisma Healthcare, PLLC does not treat cancer. Naturopathic support for immune function during conventional cancer treatment may be discussed, but cancer care falls outside the scope of this practice.

Consent to Treatment

By booking an appointment, patients consent to the naturopathic assessment and treatment modalities performed at Holisma Healthcare, PLLC, as described in the intake consent forms completed through the Jane patient portal. Patients retain the right to withdraw consent at any time. Holisma Healthcare, PLLC and Dr. Mikailah Grover, ND do not guarantee any specific outcome or improvement of condition.

Informed Consent for Minors

A parent or legal guardian must provide informed consent for treatment of patients under 18 years of age, except in circumstances where Washington State law permits a minor to consent to treatment independently.

Notice to Patients Who Are Pregnant

Some naturopathic therapies may present risk during pregnancy. All patients who are pregnant, or who suspect they may be pregnant, must inform Dr. Grover before any treatment is provided.

Governing Law

These terms are governed by the laws of Washington State.

Website Data Collection

What Data This Website Collects

The Holisma Healthcare website collects data about visitor behavior for the purpose of improving site performance and understanding how prospective patients find and use the site. This data is collected through GoHighLevel (GHL), the platform that hosts the BNS seminar registration system and marketing automations connected to this website. Future additions to the site's tracking stack will include Google Analytics and Meta Pixel; this notice will be updated when those tools are added.

Data collected may include pages visited, time on page, referral source, device type, browser type, geographic region, and IP address. This data is aggregated and does not identify individual visitors by name unless a visitor completes a form, submits a contact inquiry, or registers for the BNS seminar.

Form Submissions

When a visitor submits a contact form, registers for the BNS seminar, or subscribes to the Clinical Journal email list, the data they provide — including name and email address — is stored in GoHighLevel (for BNS seminar contacts) or Mailchimp (for journal subscribers). These platforms receive name and email only. No clinical information, diagnosis, or health history is transmitted through website forms. Patient health information is handled exclusively through Jane App, Holisma Healthcare's HIPAA-compliant electronic health records system.

This website uses cookies and similar tracking technologies to support site functionality and analytics. By continuing to use this site, visitors consent to the use of cookies in accordance with this policy. Visitors may adjust cookie settings through their browser; disabling cookies may affect certain site features.

Data Retention and Rights

Website visitor data collected through GoHighLevel is retained in accordance with GoHighLevel's data retention policies. Journal subscribers may unsubscribe at any time using the link in any email communication. BNS seminar registrants may request deletion of their contact record by emailing [email protected].

Washington residents have rights under the Washington My Health MY Data Act and applicable state privacy law regarding personal data collection. Contact [email protected] with any privacy-related requests.

TECHNOLOGY DISCLOSURE

AI & Technology Disclosure

AI-Assisted Clinical Documentation

Holisma Healthcare uses Jane App's AI Scribe feature to assist with the generation of clinical documentation during appointments. With the patient's consent, the AI Scribe creates a draft clinical note based on the appointment conversation. Dr. Grover reviews, edits, and approves all AI-generated documentation before it becomes part of a patient's official medical record. No AI-generated note is accepted without Dr. Grover's review and authorization.

Patients are asked to provide written consent for AI Scribe use through the intake consent process in the Jane patient portal. Patients who prefer that AI Scribe not be used during their appointments may request this at any time. This preference does not affect the patient's right to care or to future treatment.

Jane App's AI Scribe operates within Jane's HIPAA-compliant infrastructure. Voice data from appointments is processed to generate documentation and is not stored after the session is complete. Jane App's AI privacy practices are described at jane.app/legal/privacy-notice.

AI-Assisted Content Creation

Holisma Healthcare uses AI tools, including Claude (Anthropic), to assist in drafting website copy, educational content, patient communications, and marketing materials. All AI-assisted content is reviewed, edited, and approved by Holisma Healthcare before publication. AI tools are not used to make clinical decisions, generate patient-specific medical advice, or access patient health records.

No AI in Clinical Decision-Making

No AI tool is used at Holisma Healthcare to diagnose conditions, generate treatment recommendations, interpret lab results, or make any clinical determination. All clinical judgments are made by Dr. Mikailah Grover, ND.

Future Technology Changes

Holisma Healthcare will update this disclosure when new AI or automated technology tools are added to clinical or administrative operations that affect patient data or patient communication.
